Re: Learning Lessons to Improve Care Clinical Summit

Please can I draw your attention to this summit, which will build on the work and recommendations of the work led by Ron Hsu and Lucy Douglas-Pannett on the ‘Learning Lessons to improve Care Review’, which many of you will know as the ‘Joint Mortality Review’?
The evening should be very stimulating and includes a talk ‘Rediscovering Lost Values’ by the leadership and patient safety guru Aidan Halligan. This will be followed by group work to share experiences and ideas.
The 23% incidence of unacceptable care identified in the case reviews of 381 patients who died following admission to UHL provides a tremendous opportunity to work together to improve the care of patients in the region. The review identified specific issues including communication (particularly of do not resuscitate decisions), hydration and end of life care. We need to celebrate and champion the things that are done well in LLR and those that are improving. We also need to work together to tackle system failures that may continue to cause unacceptable care in community and hospital settings in Leicestershire and Rutland.
I appreciate that you and your colleagues are all working exceptionally hard – please consider attending this event and encourage nurse, health care assistant, medical and non-medical colleagues from your practice to attend.
